Perioperative Medicine Service

Improving outcomes, streamlining care

The Perioperative Liaison Service at The Physician’s Clinic is delivered by consultant geriatricians and designed to improve patient outcomes, reduce complications and streamline inpatient care.

As part of this service, we offer comprehensive medical assessment, optimisation and continuity of care for patients undergoing elective or emergency surgery.

We’ll review your patient’s health in detail and take steps to modify any risks before their operation, including: 

  • Multiple long-term conditions
  • Frailty and functional decline
  • Polypharmacy and high-risk medications
  • Cognitive impairment, delirium risk and mood disorders
  • Nutritional deficits
  • Smoking and alcohol-related risks

For older or complex patients, we’ll provide a structured multidisciplinary review of:

  • Physical health
  • Mobility and falls risk
  • Cognition and mood
  • Frailty and physiological reserve
  • Social support and discharge planning needs

We can offer fully personalised strategies to improve strength, nutrition, mobility, sleep and psychological readiness ahead of surgery.

  • Reduced post-operative complications
  • Fewer episodes of delirium or medical decompensation
  • Reduced length of stay and smoother discharge planning
  • Improved anaesthetic safety
  • Clearer perioperative planning for high-risk patients
  • Enhanced MDT communication
  • Earlier identification and management of deterioration

We also continue consultant-led care following surgery, reviewing patients on the ward in order to:

  • Manage post-operative medical issues (working closely with AIM and ITU where required)
  • Optimise pain control and medication plans
  • Support mobility, nutrition and hydration
  • Identify early complications
  • Reduce delirium and functional decline
  • Support discharge planning and rehabilitation

This continuity of consultant care supports a safer recovery and helps reduce avoidable readmissions.

Who you should refer

Our service is suitable for patients undergoing elective or emergency surgery who have:

  • High predicted perioperative risk
  • Multiple comorbidities
  • Frailty or mobility issues
  • Polypharmacy
  • Cognitive concerns, memory impairment or delirium risk
  • Complex medical, functional or social needs
  • Difficult decision-making around surgical management

While we encourage early referral, we are able to offer support at any stage of your patient’s surgical pathway.

How to book an appointment

How to refer to the Perioperative Liaison Service

We accept referrals from:

  • Outpatient clinics and MDT meetings
  • Pre-assessment clinics
  • Ward teams for post-operative review
  • GPs

The named consultant surgeon must be aware of and approve all referrals.

All direct referrals should be followed by:

  • A referral letter
  • For inpatients, documentation in the clinical notes

Perioperative consultants respond promptly and work closely with surgical, anaesthetic and ward MDTs.

Important: Any new acute medical presentation or deterioration in a surgical inpatient should be referred to the AIM on-call consultant at The Princess Grace Hospital.
